Project Start form 0.3.1. The rent help follow-up no longer offers "Client doesn't know," "Client prefers not to answer," or "Data not collected," because HUD gives that list none (Data Dictionary p.164).

Its guidance now says what to do when the person does not know, in the same words as the Project Exit form.

New: Project Exit form, draft 0.1.0. For every project type, one form for each person who leaves.

• The exit date, with the rule for each kind of program: the day after the last bed night for night-by-night shelter, the last contact for street outreach, and the last service for rapid re-housing (Data Standards Manual p.25 and p.64 to 65).
• Where the person goes next, with all 29 answers HUD allows for Destination in Appendix A, grouped the way the Appendix groups them. The follow-up about the kind of rent help shows, and prints, only when "Rental by client, with ongoing housing subsidy" is marked. "Other" has its own box to write where.
• Money coming in, other benefits, and health insurance at exit (4.02, 4.03, 4.04), worded like the Project Start form. The guidance says the exit answers are a new record, not a change to the answers from entry.
• What to mark when a person left without notice, died, or is going somewhere not on the list, and what to do when people in one household leave on different days.
• The form says which exit questions it leaves out, and why.

The site. The checks that compare each form with the FY2026 Data Dictionary, and the checks that compare the screen with the printout, now cover both forms.

Project Start form 0.3.0, and the site:

• The disabling condition question moved to its own last section, "Health and disability," apart from the choice to take someone in, as Fair Housing law requires (Data Standards Manual p.61). What the person hears and what is recorded did not change.
• When "Rental by client, with ongoing housing subsidy" (435) is marked, a follow-up asks what kind of rent help it was, with HUD's 11 subsidy types (Data Dictionary p.164). It shows, and prints, only when 435 is marked.
• The income guidance now says where money from family or friends goes: it is not one of HUD's sources, so money that comes in regularly is marked Other source.
• The project start date guidance now says a street outreach start date is the date of first contact, which can be earlier than the day the form is filled in.
• "Flag this question" now opens the feedback panel set to "A question is confusing," and you can still pick another. The Feedback button still opens with nothing chosen.
• The stylesheet is half its old size: rules for screens this site does not have are gone. No page looks any different.

Project Start form 0.2.0. Every question was checked against the FY2026 HMIS Data Dictionary and Data Standards Manual, and each mapping now cites its page. The length-of-stay guidance was rewritten in plain words after a field report said "round honestly to the band" made no sense: it now says the choices are HUD's time ranges, that the person's own guess is fine, and explains the 90-day rule with an example. The veteran question no longer counts basic training alone as service, which the Manual counts as No. Income asks about money coming in now, and the source names match the Dictionary word for word. Nothing any question records has changed.

The site. A text size button (100, 125 and 150 percent) that scales every page, the form and the printout. A simpler feedback panel: the box to write in comes first, the category is optional, and a failed send keeps what you wrote. Every "show more" control now says what it opens. A plain-words summary on every page. The navigation is repeated at the bottom of each page. Private browsing no longer breaks any setting.

Reliability release, no form content changed. Error monitoring (Sentry, error reports only: session replay is switched off at the source, and browsers that send the Global Privacy Control signal are never enrolled). Weekly automated accessibility scans and a screen/print parity check now run against the live site: the check renders the Project Start form and verifies the page shows exactly the questions the definition contains.

Screen and print render from one definition. Feedback flagging on every page and every question. Draft status: nothing here is certified; review before local use.
